Informed Consent and Participation Agreement

Important: This informed consent agreement uses plain language and is intended for participants in different countries. No single online form can guarantee enforceability in every jurisdiction. Nature and scope

I understand that Rusty Diamond provides educational hypnosis, guided imagery, relaxation, consciousness exploration, creativity work, and related self exploration. These services are collaborative and individualized.

I understand that these services are not medical care, psychotherapy, psychiatric care, diagnosis, legal advice, or emergency support. Rusty does not promise a particular result.

My choices and responsibilities

I am participating voluntarily and may pause or stop at any time. I understand that I remain responsible for my choices, actions, and wellbeing. I will not participate while driving, operating machinery, under the influence, or in an unsafe setting. For an online session, I will use a private, stable location without distractions and provide my physical location at the start of the session. I will disclose relevant health conditions, medications, current treatment, pregnancy, seizure history, recent crises, or other concerns that could affect safe participation. I will seek appropriate licensed medical or mental health care when needed.

Memory, regression, and unusual experiences

I understand that memory is reconstructive. Images, emotions, impressions, and experiences arising during hypnosis may feel vivid without being historically accurate. I will not treat hypnotic recall, dream content, past life material, or imaginative experience as verified fact without independent evidence.

Possible effects and limits

I understand that hypnosis and focused self exploration can sometimes bring up unexpected emotions, memories, physical sensations, fatigue, temporary discomfort, or no noticeable change. I agree to communicate discomfort and use my right to stop.

Privacy, technology, and recordings

Reasonable care will be used to protect privacy, but internet and electronic communication can never be guaranteed completely secure. Sessions will not be recorded by Rusty unless separate written permission is given. I will not record or publish a session without Rusty's written permission.

I understand the privacy and technology limits of remote sessions.

Fees, scheduling, and cancellation

I understand that a refundable consultation fee is required to reserve a consultation, the full session fee is required before booking a session, and the stated payment total may include processing costs. Specific cancellation, rescheduling, and refund terms must be provided before payment.

Release and applicable law

To the extent permitted by applicable law, I accept the ordinary and disclosed risks of voluntary participation and release Rusty Diamond and Hypnosis Is Great from claims arising solely from those ordinary risks. Nothing in this agreement excludes liability that cannot legally be excluded, including liability for fraud, intentional misconduct, gross negligence, or consumer rights that cannot be waived.

Age and capacity

I am at least 18 years old and have legal capacity to consent, or I have contacted Rusty for a separate parent or guardian process before booking.
